What does the Annual Membership Fee cover?
In the Women's Health Centre some of the services we provide are not covered under the OHIP program.
The fee for managing these services is $150.00 each year.
It is payable by CASH ONLY - non-refundable, to be paid on the first visit.
Uninsured services are included in the fee:
There are some uninsured services not included in the fee:
i.e. Travel advice, S&A forms, TB tests, 3rd party physicals, Driver's physicals, copy of medical record.
They would be paid for on an "as needed" basis. |
